STARTING SERVICES
After you submit a request for services you will be contacted by our office manager. She will gather some basic information in order to send you instructions on how to complete your intake forms online. You must complete the new client intake forms in advance of your first appointment or it will be cancelled.

These forms include information regarding insurance benefits, payment methods, and critical information your therapist will need to work with you effectively and help you reach your treatment goals. Please take the time to be thoughtful in your responses and review the documents to understand your rights as a client.
